This form is required for ALL exchanges. We will contact you within 1-2 business days with instructions on how to make your exchange. Please wait for proper return address and instructions. We do not offer return refunds.


EXCHANGE RULES & CONDITIONS:

  1. Items must be returned in NEW condition, free of hair and smells (dog + smoke)
  2. Items must be returned with tags still attached
  3. All custom items are not returnable
  4. All items marked Clearance are not returnable
  5. All jewelry + hair accessories are not returnable
  6. All return requests must be made within 14 days of receipt
  7. My Formal Dog offers size exchanges for the same item.
  8. ALL exchanges are shipped USPS First Class. If you would like your item shipped USPS Priority please include $8.85 with your return.
